Welcome to Your Patient Portal:
This is the most efficient way to contact the office.
We check the portal a minimum of 2x a day Monday-Thursday.
On Fridays through Sunday we may check daily but sometimes are not able.
It is most helpful if you request your prescription through the PORTAL. Please do NOT assume the pharmacy has reached out to us, especially if they say we are not returning their messages.
Appointments are needed at a minimum every 6 months or a year. Prescriptions will NOT be renewed if an appointment is not on the schedule within a reasonable amount of time.
For
class II (stimulants) or class iV (benzodiazepines) medication refills please give us 48 hours lead time so as not to run out.
Also,
1. please list your preferred pharmacy. If you choose it we know it is correct. There are multiple Walgreens/CVS/Osco on the same streets and towns.
2. Please check your information, especially preferred telephone numbers, email and home addresses.
